The Morning Toothbrush!
How many of us hate our gooey mouths and brush our teeth first thing in the morning? Many, I am sure. Last night, during a chat, Deepak told me that he had just woken up, brushed his teeth and had his chai. My immediate response was to ask him why he hadn’t had his chai first and then brushed his teeth? To this he asked me, quite surprised, if I had my chai before brushing my teeth. My answer: yes!
Most of us, as a part of growing up, are taught to brush our teeth as soon as we wake up. Why? Have you ever asked yourselves? It is pretty obvious, y0u might say. The mouth smells bad due to all the bacterial action from the night before and the teeth need to be cleaned for a fresh mouth first thing in the morning. True I say, but does it make sense to clean our mouths first thing in the morning and then immediately proceed to have breakfast, thus reinviting all the bacteria that we had just brushed away?
If the point of brushing was to have a fresh breath and removal of foul smell, then any mouthwash would do. However, the point of brushing, as I see it, is to clean the mouth completely - to free our teeth from food particles, plaque, and bacteria to maintain a healthy oral environment. So if that is the point of brushing, what is the point of eating breakfast right after that morning brush?
All my life I grew up with this routine - wake up, brush, chai/bournvita, a little breakfast and off to school. Kids nowadays also brush their teeth at night. Great! However, after studying and pursuing dental hygiene, and after several debates on when to brush in the morning, I have started advising patients, friends, and family to brush after breakfast. I would like to present my argument here!
Our mouth has a dynamic environment and harbours more bacteria, good and bad, than you can ever think of. Eating different kinds of food constantly change the acidity in our mouths and as food gets trapped in between our teeth, it invites bacteria to settle there and multiply. Sugars produce an acidic coating over our teeth which make the enamel of our teeth susceptible to decay. After food is trapped for a few hours, it turns into the white sticky goo that you see in the morning called plaque. Foul smell also sometimes arises from the plaque staying put in the same place for longer periods of time. The tongue constantly cleanses our mouth but we also produce saliva which can be both good and bad. Good because when we talk and chew, saliva acts as a self cleanser. Bad because when we are asleep, this still saliva acts as a sort of glue for the plaque to stick even harder on our teeth - to put it simply. To keep our teeth and gums healthy and free of such decay and disease causing bacteria, it thus becomes imperative to keep our teeth free from such food debris, especially those that get stuck in the tiny spaces in between our teeth. How do we do it? Brush and floss.
We eat different meals throughout the day. Imagine the amount of acid and sugar production going on in our mouths the whole day - undisturbed till we brush at night. For some of us, till we brush the next morning. We wake up the next morning and brush to disrupt the little colonies that they have formed. Then we have breakfast. And then we proceed through the whole day eating - not worrying about the mini-party these bacteria are having in our mouths.
If you feel the strong urge to brush right after you wake up because you cannot stand the gooey feeling, then do a quick rinse with water. However, brush only after you have had your breakfast. Imagine cleaning your teeth and then sticking those bread crumbs, or paratha, or bagels, or omlette, or even the sugars in your chai in between your teeth. What a nice layer of fodder for bacteria! Brushing after this morning tea or breakfast will ensure that your teeth remain clean and free of food, thus less fodder for bacteria to multiply. As simple as that!
I remember telling little kids that if they did not brush at night, the germs will attack the food from the whole day and have a party in their mouth throughout the night. Sleeping with a clean mouth ensures that bacteria that now have a still environment without all the tongue and saliva movement do not get much fodder to multiply. So brushing at night is also important. But if you MUST brush only once a day then after breakfast is the way to go.
Moreover, brushing after breakfast ensures that the fluoride and therapeutic substances in your toothpaste have a clean sugar free enviroment to act on - atleast till lunch time
If you do not see light in this little argument do attack me with questions. While I am not a licensed dental professional yet, and this might not be expert advise - I do believe in educating the public! If ya know whats going on in your mouth, you will be in a better position to make an informed decision about when to brush your teeth! If this was confusing as heck or if you have a better argument against brushing teeth after breakfast do let me know - I will check my facts up.

Kaush Aunty Kaush Aunty, Jab se you told me (around one year back) to brush my teeth after breakfast, believe it or not I do it after breakfast. Although, sometimes I have my first meal mid-day so you can very well imagine what my roommates have to go through.
I also brush after lunch (if I am in Univ then I have that dissolvable thingies) and ofcourse after dinner. My mom used to make sure that I brush after dinner and she would sometimes even wake me up and make me do it while I would be half-asleep. Ab aadat se ho gayee hain, if I sleep while taking a nap, I would get up at 2-3 am, brush and sleep.

Yep me brushes at night…its now a habit that’s gonna live. In fact its so ingrained, that if sometimes i doze off in front of laptop, i wake up in the night feeling uncomfy, and then brush and then pack off again
Also, the chai bit is ok, as I told u often i do that before brushing…but breakfast is a bit hard to digest
This post made me remember of my biology class in school , bacteria and stuff
I brush my teeth before breakfast as we were taught to do so..this fact is new to me, never knew abt that and 1st time I m hearing it. I am hardly in that field, so will jus keep my ears open to advice from Doc’s like Kaush

I have always been taught to brush my teeth to feel fresh after waking up. I am very strict on this matter and seriously will not eat until I have brush my teeth.

Deepak, haanji silver aapka..and haanji maine post kar hi diya! I was just itching to post right after our little conversation! Good good, you too have the habit of brushing at night! Thats the best thing you can do to protect against cavities! try it for a few days and I am sure you will get used to brushing after breakfast too
JS, I used to brush first thing in the morning too until recently. and LOL, this post was a lot like bio class ..sahi hai! Its a great habit to brush at night but try seeing the reason behind this, and only if you see reason brush after breakfast!
Manpreet Sahi hai! If you brush your teeth first thing in the morning but dont have any breakfast then I suggest you do have breakfast because that is the most important meal of the day. And then, try brushing after breakfast everymorning. Like I said in the post, if you absolutely cannot stand your teeth feeling gooey and smelly then rinse your mouth..or even brush it lightly..but dont forget to brush after morning nashta!

Poonam dear That is the POINT of brushing….to remove bacteria harboring debris and food! and American?? LOL yes yes it is true! I used to say that too….even B’s mom said..yeh america se aakar mere bete ko american bana ke gai - woh bhi breakfast khaane ke baad brush karta hai! But thats the point I am trying to make. Ke khaane ke baad hi brush karo toh brushing’s main goal is accomplished!
And you are very correct. In an ideal world, one should ALWAYS brush after every meal. Thats the way to go! But sadly thats not the way we all live. Not even the best of dentists, because it is sometimes simply not feasible. So, you try to do as much as you can to maintain a healthy mouth! Brush after breakfast. I know subah subah uthkar our mouth smells and doesnt feel clean..so like i said in the post - do a mouthrinse and lightly brush if you feel its extremely bad, but always try to brush after breakfast. Usually we dont have anything else to eat for atleast 4-6 hours after breakfast …..and if we have brushed our teeth…there is very little fodder for bacteria to multiply. think of this as wiping your dining table. Khaane se pehle hi saaf kar diya toh khaane ke baad dirty rahega jab tak next time clean naa kiya jaay - just an analogy. Try it poonam dear and I am sure you will see the difference. Also, it feels good to leave home with a clean, fresh mouth.

Mehak, sahi hai! Good way to go. I do the same. Waise sometimes you just do the best you can. Your RCT is probably a result of decay over a looong period of time that went unchecked. But keep it up! Since you do all this..how bout one more advise, start flossing

I had one small doubt .. animals dont brush at all .. dont they get any dental problems … I have observed some buffalos and dogs , their teeth seems to be shining where as most of the humans (including me) who brush regularly have their enamel layer disappearing … whats this difference and why ????

getting serious, i understand your logic and you are definately qualified to give this advice so cannot even deny it but, irrespective of that I cannot imagine eating/ drinking before brushing. I am sorry I just cant, the reason is that the first thing I do in the morning after brushing is drink 250 ml of water and start cooking breakfast cum lunch and then after almost 2 hours I have breakfast which contains of coffee and sandwichs or biscuits or leftovers so effectively you are telling me that I should keep the foul smelling mouth uncleaned for 3 hours afer I wake up??? that is quite yuckkky actually. I understand when you say that you can rinse the mouth right after waking up but it is not the same. there is nothing like a good strong brushing early in the morning.
In short doc, tell me what I should be doing? this brushing after tea/ coffee doesn’t work for me coz i will miss my ladies special in your brushing business.

lol ok ok maam… but just the idea of putting somehting in my mouth without brushing is yuck…i dont even talk without brushing in the morning! lol
hi deepak, i get up at 6:00 go for walk, cook, clean the house, wait for the bai who comes at around 8:00 then catch the 8:37 ladies special. that is almost 3 hours na? I have breakfast at around 8:00 so 2 hours of dirty mouth. eeekkksss. 3 was an exageration but 2 is too eeewwwyyyy too.
kaush- i was quite surprised when my kids’ dentist told them to brudh after b’fast. i make sure they brush befoe and after breakfast now and so do i and then at night..so we are constantly brushing our teeth! like everyone here, the thought of eating or drinking w/o brushing grosses me out a bit. just what you grow up accustomed to, i guess- RS

if we are not ideal dentist patients.. which you have agreed to:P then this is just part of that..what about overnight bacteria?? is there no such thing? LOL. well now think of it this way..for those of us non-ideal dentist patients who don’t even brush at night **gasp**…wouldn’t we have overnight bacteria..which would then grow..and get dug deeper once we eat something…? by brushing before we eat..we get rid of that nasty over-night bacteria.

Prakash Thanks Prakash. Actually I have been asked that question for the first time. My best guess is that animals could have a thicker enamel than us humans. Also, humans eat a whole lot of modified foods while animals mostle feed on less modified and more natural foods. Also keep in mind that white teeth are not necessarily indications of a healthy mouth. sometimes a persons smile is very white but from the inside the gums are damaged and there is bone loss and lots of bacteria! There’s my best guess.
Also, I knew this dog who had so badly damaged his teeth that a doctor had to pull five.

Chandu Yes actually. Some toothpastes are more abrasive than others. Abrasive material is included in toothpastes to remove stains which in turn makes teeth white. Sometimes, aggressive use of these toothpastes combined with hard brushing or brushing with a toothbrush with hard bristles will lead to loss of enamel over time. And LOL…yea I had to convince my mom too and she wouldn’t hear of it..but I still did it
Priyangini I, too, like all of us here was accustomed to getting up and brushing my teeth! That was like a natural reflex for me too and just the thought of brushing after breakfast was eeewwwy! That habit is so ingrained that even now if I wake up out of deep sleep,I just walk to the bathroom and brush..but then always end up brushing again after breakfast. Mera routine kuch similar sa hai - I wake up, rinse, and also drink a whole glass of water…and the rest but the only thing differnt brush after.
You dont have to do it coz you are entitled to make that decision for yourself and you know the logic behind it and its totally your opinion after that. And as to telling what you should be doing? Well….if you absolutely dont want to brush after then you dont have to. As to getting rid of the plaque from breakfast, a quick fix before leaving so that you dont miss the ladies special could be just to use the toothbrush sans the toothpaste and brush your teeth for a minute before leaving, maybe? If time doesnt permit…no big deal. btw do ya brush your teeth at night?
Speaking of ladies special…I was quite surprised this past August when I got on one. I had no idea there was a ladies special too!BUt on the train during the rush hour is still a struggle!
Galnextdoor I used to be like that. Even now, I have to rinse with listerine or whatever free rinse i get from the dental school first thing in the morning.
RS Righto! I was accustomed to the same thing. Actually everytime I am in clinic at school, I actually end up brushing after lunch too….sometimes 3-4 times a day! hahahaha…..but once it becomes a habit, its not too hard to keep up

Sahi hai Poonam ji you don’t have to agree at all! And as to what about overnight bacteria? Brushing at night! Thats actually the number one thing protecting your teeth against bacteria. and true, those who dont brush at night will have a lot higher bacteria in the morning and its true eating before brushing will increase the bacteria, but not to the point that it will be that hard to remove coz then you brush soon after. And what you said is true—by brushing before we eat, we get rid of the overnight bacteria. However, by eating right after, we also override that brushing coz then we get rid of one set of bacteria and soon after put in more food for a new set. All in all, its up to each person…we are not all ideal..but the idea is to make little changes and do what we can and use our best judgement. I too was soo grossed out by the thought of not brushing first thing…and it was gross for a while..but I feel more comfortable now. and so whatever makes you comfortable goes!

Q1. How long should you do brushing?
Reason: My cousin used to do it for 10 min. but I hardly do it for less than 3 min. but my teeth are very sensitive, so I don’t brush too long.
Q2. Is there any good recommended toothpaste for sensitive teeth because I can only use “Aim”, rest don’t suit me.
Q3. Is electric brush better because it doesn’t suit me either and the first and last time I used, I thought all my teeth would fall off…LOL
Q4. What other things you can recommend for healthy teeth, like food…any special kind or flossing or one of my gujju friend used to clean his tounge too, what was that all about with that steel thingy?
Q5. What about teeth-whiteners? Are they any good or just a cheap gimmick??

1 - You should brush for atleast 2-3 minutes. Actually, it does not seem too long but when you actually time yourself it ends up being pretty long. Average time a person brushes is about 33 seconds! but it should be 2-3 mins ideally
2 - Rest dont suit you bole toh? They dont go with your suits? or you dont like the taste? or they give you bad reactions? Anyways…Sensodyne is a good one..am not sure if they have it in Canada, also crest sensitivity toothpaste is a good one. In fact, any toothpaste with a high potassium nitrate content and a high stannous fluoride or sodium fluoride content are good. Fluorise mouth rinses can also be beneficial - ACT rinse is a good one.
3 - Research has shown no major difference between manual toothbrushing and power toothbrushing. If a person has arthritis or low manual dexterity, then yes…power is better..but for those who have no difficulty using a manual..research does not indicate a statistically significant difference. I personally dont have a preference either coz I like both! Power toothbrushes can work better for some people though.
4 - Flossing is great anytime! And yes cleaning your tongue is very important. Actually, I have been cleaning my tongue ever since I was a child and I thought the “steely thing” that you saw in your Gujju friend’s hand was all over India!! I did not kmow you did not know about it. Actually the tongue harbours a lot of bacteria, also part of the bad breath when we wake up actually comes from the bacteria on the tongue. You dont have to use the steel tongue scraper but you can use your toothbrush to brush you tongue. In America, more dentists are now recommending tongue brushing….plastic scrapers are available in stores now
5 - Tooth whitening agents are available in a wide variety. usually the reputed crest whitening strips are good. The difference however between professional tooth whitening systems and otc(over the counter) systems is that professional systems last much longer. Lots of details to get into with whitening agents..sigh
Also, if you have sensitivity its not a good idea to start whitening coz they can increase the sensitivity. Start only after you have less sensitivity.

1. Yea, I brush 2-3 minutes with a timer.
2. Rest don’t suit me bole toh my teeth bleed if I change my tootpaste. I like toothpastes that floss a lot like we used to have in India. Here, the toothpastes are different, you don’t get too much floss (or watever the white stuff you dentist-geeks call it). Abey oye, what do you mean if we get in Canada, we get everything here…LOL.
3. Ok, I will stick to manual as if now.
4. Nobody in my family uses that tounge thingy. I will make sure I brush it from now on. Don’t remind me of my Gujju friend and that tounge thingy, there was a controversy rergarding that…LOL.
5. I use tooth-whitening strips for special occasions (like date-shate…wink wink). Tabhi poocha tha…:D

Ricky Kaahe ka phokat haaan? gujjuness is inherent…can never go away! waise you are confusing floss with FOAM ricky! Floss is something totally different from foam/froth. and your gums bleed!!!!!!! That is NOT a goood sign..i told you before too….thats gingivitis if your gums bleed when you brush! start flossing and lets chat sometime so that I can give ya a piece of my mind about going and seeing your dentist ASAP!

I mentioned tht my dental practice is bad.. cos I hv a cavity in one of my tooth for a long time and the teeth seem to go off slowly.. and now jus some pieces of tht remains.. Am jus very much afraid to go to the dentist
But, i don’t use any of tht mouth freshner or don’t brush at night.. 

And reg brushing, i brush after I get up in the morning and then i also use the tongue cleaner thing.. the steel thingy as Ricky said

Oh boy Dr. Jaush in action! Never seen things being discussed at this length and with this level of seriousness on blogs. But must admit, this indeed is a useful bunch of information. Most of us tend to take things like brushing for granted na? The real worth of dental hygiene is understood only after you visit a dentist once and get those pointed (and of course dangerous looking) things poked in the mouth! :O) And besides you end up shelling out fortune too!! Thanks to noble souls like Dr. Kaush for this patient consulting to the souls like us who are not-so-noble to our teeth!! Prasad phew! those are a lot of questions…lemme try to answer them
1. yes
2. well, I did not say you HAVE to brush your tongue with the brush> I always brush my tongue with teh steel tongue scraper…and I have not heard anything about tastebuds being damaged so am not sure on that one. But I myself use the steel one..so its really up to you. ricky did not like it so i gave him an alternative.
3. Dental floss is essentially a nylon mint coated thread available in stores. This thread is used to remove tiny food debris from in between the teeth which are more susceptible to food particles being stuck for a long period of time. Actually, when I was in Bangalore, I did see Oral B Satin Floss in FabMall at the Forum. Just google flossing and you should come up with all kinds of flosses and how to floss. Lemme know if you have nay more questions about that.
4. I am always a little wary of whitening agents because strong chemicals do have a tendency of making your teeth sensitive. However, if you think that in 10 yrs or so your teeth will be yellow then you can use whitening agents then. For right now, make sure you dont brush TOO hard and take care with drinking lots of soft drinks, and sugary juices - that should help reduce the “yellowing” or wasting away of the enamel.
As to your cavity…sounds like your tooth is wasting away and it might not be too far from getting worse> Dont be afraid to go to the dentist. think about it this way, the sooner you get it done - the easier it will be to treat it. Usually dentists will give you anesthetic and it is all numb. remember, a cavity is an infection and has particular bacteria that cause cavities. Just like any infection it is infectious! and canbe passed on from one tooth to the other….so make sure you get that tooth taken care of pretty soon to avoid problems with other teeth as well. I recently was in east asia and I noticed it is a norm there to keep (or carry) a toothbrush and toothpaste in the office and brush teeth immediately after lunch (poor overworked bastards have food delivered to the office). Now I don’t know whether it’s done for hygenic, cosmetic or just feel-good reasons, but I really liked the idea. Now, back in europe, I am stuck with a sweetened-with-xylitol-only chewing gum as a meager substitute for a post-lunch skirmish with the plaque. I wonder if it does much good…
